SEVERE WEATHER CHILD CARE

How to Apply

Eligible staff can apply for the Severe Weather Child Care Program by completing the following steps:

  1. Pre-register: Space at locations is limited, so it is necessary for staff to pre-register for the program. Pre-registration includes submitting a complete pre-registration packet, which consists of providing your contact information, your child's immunization records and health information, and emergency contacts. The packet is accessible through the site coordinator or online. Pre-registration alone does not reserve space for your child during severe weather.
     
  2. Reserve space: When severe weather is imminent, pre-registered staff will need to reserve space for their child by calling a special Severe Weather Child Care RSVP line. Child care will be available on a first-reserved, first-served basis for those who have pre-registered and called the Severe Weather Child Care RSVP line. You may leave a message up until 11 p.m. each night of the severe weather period to reserve space for your child for the following day. Reservations will not be accepted after 11 p.m. If no requests are made by eligible staff to reserve space at one of the designated locations, child care will not be offered at that location.
     
  3. Review and update records every six months: Parents are required to keep the pre-registration information current. DUHS will publish reminders in November and May of each year to ensure that records are updated in time for the winter storm and hurricane seasons. Parents may choose to complete new forms for each child or they may simply complete the Bi-annual Update Form

Pre-Registration Process

Follow these steps to pre-register your child:

  1. Confirm with your supervisor that you are in an Essential Service job as defined by Duke's Severe Weather & Emergency Conditions Policy to determine your eligibility to apply.
     
  2. The pre-registration packet includes:
  • Pre-Registration Form
  • Release Consent Form
  • Emergency Medical Treatment Consent
  • Immunization History (may be completed by doctor or substituted with a medical history form from your doctor)
  1. Return a signed, original version of the pre-registration packet to your site coordinator. Faxed, copied, unsigned, or incomplete forms will be returned and the child will not be pre-registered until completed forms are on file.

Make A Reservation When Severe Weather is Imminent
