These ranking/feedback forms are examples of types of evaluation documentation which interviewers, search committees or panels should employ in summarizing the interview process. It is especially important when several individuals are conducting the interviews to have a standard format for all to use to avoid bias in the evaluation phase of the interview.
Example A enables interviewers to evaluate each candidate in reference to a single set of behavioral interview questions used in the interview. Example B demonstrates a format for evaluating the candidate on pre-determined values and competencies which the hiring official has deemed as essential to success for the candidate. Items can be evaluated and weighted as to their relative importance in the interview process as in Example B.
Whatever format is used, interview notes and feedback forms should be maintained in departmental files for 3 years for every position that is filled within the department.
