Don't Wait! Submit a
New Pledge for 2008!

Due to new IRS tax regulations, a pledge must be submitted each year to continue your pledge.

 

Donors must submit a new pledge form each year. Please retain a copy of your completed pledge form for tax purposes.

Print a pledge form and submit it via campus mail to Box 90520.

Stopping Your Pledge

To discontinue your pledge to the United Way Duke Partnership campaign, just fill out this form. Cancellations take effect based on corporate payroll deadlines. Please email your cancellation request to sandfp@duke.edu.
