Parental Leave Procedure
To apply for Paid Parental Leave:
- Complete a Paid Parental Leave Request/ Primary Caregiver Affidavit
- Complete an application for Leave of Absence Request form.
- Have your department payroll clerk complete a Leave of Absence form
To insure accurate and timely payment, the employee is responsible for
submitting all of these forms together to Staff & Labor Relations, HR, Duke
South, Blue Zone, Room 1527, Box 3904 at least 30 days in advance of the first day of leave. The Paid Parental
Leave will run concurrently with the associated Family Medical Leave.
There shall be a consecutive three week waiting period commencing from the last
day worked before this benefit will go into effect. The staff member may use any
or all of their accrued sick leave, vacation, or PTO or take time without pay
during the 3-week waiting period. Payment for Paid Parental Leave will be paid
on regular payroll dates.
