April 15 Deadline to Submit 2004 Reimbursement Account Claims
Participants enrolled in health care or dependent care reimbursement accounts for the 2004 plan year (January 1, 2004 - December 31, 2004) should submit all claims by April 15, 2005 to request reimbursement.
In order for eligible expenses incurred in the calendar year 2004 to be reimbursed, completed claim forms and supporting documentation must be post marked or faxed to Aon Consulting by April 15, 2005. Claims for 2004 submitted after this date cannot be reimbursed.
Please submit completed claim forms and documentation to Aon Consulting by fax at 336-728-2981 or 336-728-2983, or U.S. mail to Aon Consulting, Flex Administration, P.O. Box 2845, Winston-Salem, N.C., 27102-2845.
Reimbursement account claim forms for the 2004 plan year can be found
online, the Human Resources office at
705 Broad Street, or by contacting Aon. For information about your reimbursement account balance or questions regarding 2004 claims, contact Aon at 1-800-726-3221, Monday through Friday, 8 a.m. to 6 p.m., EST.
