You Could Win $1,000 in Groceries For Participating in Duke Prospective Health

Duke Prospective Health. More than 2,300 Duke faculty, staff and their family members currently participating in Duke Prospective Health are now eligible to win $1,000 worth of gift cards to Harris Teeter grocery stores through a new "Giveaway Program" being introduced by Duke Prospective Health.
In the last month alone, more than 600 Duke faculty, staff and their family members have taken the first steps toward healthier living by signing up to participate in the program. Duke Prospective Health was introduced earlier this year as a means of identifying health risks and preventing diseases and chronic conditions such as hypertension and diabetes before they happen. It is offered to faculty, staff and their family members who participate in either of two health plans, Duke Basic or Duke Select.
Through the Giveaway Program, $1,000 worth of gift cards to Harris Teeter will be awarded to one participant per month from August to December 2004 through a drawing of participants. All eligible Duke faculty, staff and family members have to do to enroll in Duke Prospective Health and become eligible for the Giveaway Program is complete a simple health questionnaire and select a primary care physician, if they have not done so already. To qualify for the first drawing in August, participants must complete both the health questionnaire and select a primary care physician by July 31, 2004.
