April 15 Deadline to Submit 2007 Reimbursement Account Claims
Participants enrolled in health care or dependent care reimbursement accounts for the 2007 plan year (January 1, 2007 - December
31, 2007) should submit all claims by April 15, 2008 to request reimbursement. In order for eligible expenses incurred in the calendar
year 2007 to be reimbursed, completed claim forms and supporting documentation must be post marked or faxed to WageWorks by April 15,
2008. Claims for 2007 submitted after this date cannot be reimbursed.
Please submit completed claim forms and documentation to WageWorks. Claim forms can be found
online (see below) or at the Human Resources office at 705 Broad Street.
