Duke to Distribute 2005 W-2 Forms

The 2005 W-2 forms for all active hourly-paid staff will be mailed to the department payroll representatives no later than Tuesday, January 31, 2006. W-2 Forms for active monthly-paid staff and faculty will be mailed to the campus check mailing addresses. Department payroll representatives will distribute the W-2 forms upon receipt. W-2's will not be available prior to January 31. W-2 forms will be mailed to the home address for any faculty or staff who terminated employment with Duke before the end of the last pay period in 2005.

In an effort to better serve faculty and staff, Duke has standardized the process for requesting duplicate tax forms. Effective February 1, 2006, an employee will be required to complete a Tax Document Request Form to request a replacement of a lost or misplaced W-2 form. This form is available on the Corporate Payroll Services web site. This change is necessary to obtain an original employee signature authorizing the request. The fees associated with this request are stated on the Tax Document Request Form and remain unchanged from prior years.
