Duke faculty and staff can update Federal and North Carolina tax withholdings amounts online using the Duke@Work self service web site. |

If you are unable to update your withholding allowances online, you can find printable forms below. Tax Withholding Forms should be sent to the Payroll Office at 705 Broad Street, Box 90484.
Note: Income received from the Duke disability plan is taxable for federal and state taxes.
