Tax Withholding Forms
W-4 Form (Federal)
Employee's Withholding Allowance Form (federal). Users with Adobe Acrobat
(4.0 or higher) can fill in this form electronically. This form requires an
original signature. The W-4 Form should be sent to the Payroll Office at 705
Broad Street, Box 90484.
Note: Income received from the Duke disability plan is taxable for federal taxes.
NC-4 Form (State)
