Secondary Job Classification Request Form

Complete sections I and II of this request providing all information pertinent to the secondary job being established.  This includes a complete description of the work to be formed on the secondary job.  Requests should be sent to the Rewards and Recognition department - 705 Broad Street, Box 90514.  The requestor must have the employee sign Section IV.  Attach this form to the Personnel Change Notice (PCN) and obtain appropriate budgetary signatures prior to submission to Rewards and Recognition.
