Cost Distribution Form

To record information when an employee is transferring from one department to another or for changes in the employee's funding source, please use iForms. Instructions for using iForms and submitting online forms are included in documentation on the iForms Web Site.

If your department has not yet begun using iForms, please contact the iForms team immediately at r-3@duke.edu.
