If you have computer access along with a Duke netID (this is what you use to do the training modules) please complete and submit the Web form of the Health Review. This way is the fastest and most secure.
If you do not have access to a computer you may print and complete the PDF form of the Health Review and fax it to EOHW at 919-681-0555.
