To cancel your membership with the Duke Fitness Club online, please click here to submit your cancellation form. If you are unable to use the online format, please complete the following steps.
Please note that the LIVE FOR LIFE Duke Fitness Club contract requires at least a three-month commitment to a gym. You must
fulfill this time commitment before you can cancel your membership.
To cancel membership with the Duke Fitness Club, please complete the following steps:
Step 1 - Complete cancellation form
You will need to complete a cancellation
form and return it to LIVE FOR LIFE.
Step 2 - Submit form
Send your completed cancellation form to LIVE FOR LIFE through any of the following options:

Step 3 - Confirmation
A LIVE FOR LIFE staff member will contact you when
