Lifestyle Change Programs

If you have high blood pressure or cholesterol, it is important to manage it. The best strategy may be to make lifestyle changes — eating healthy, exercising regularly, controlling your weight and managing stress.

Pathways to Change

Pathways to Change is a targeted 12-month healthy lifestyle program for individuals who are at risk for health problems. These can include heart disease and/or stroke due to the presence of high blood pressure, high blood cholesterol, a body mass index of 30 or more, or who have pre-diabetic conditions. The PATHWAYS TO CHANGE program is a stage-based program that works with participants to help them choose their path to improving their health by lowering their key risk factors.

Steps to Health

Steps to Health is an incentive program offered by LIVE FOR LIFE that focuses primarily on helping you achieve measurable health results and rewards you for achieving your health goal. Through the Steps to Health program, you will have the opportunity to make realistic and measurable changes to your health by creating a step-by-step plan for healthier living with guidance from a LIVE FOR LIFE staff member.
