As a Duke Select or Duke Basic participant, you could be eligible to receive up to $450 in pharmacy credits through the DukeWell program or win monthly drawings for a health-related prize. Please submit this form and a DukeWell representative will contact you within 5 business days to describe what program awards you are eligible to receive.

Welcome to DukeWell
DukeWell can help you improve your health and well-being by evaluating your current health status and helping you to create a personal health plan. You'll be connected to resources to help you meet your health goals. The program focuses on preventive health services and especially on cardiovascular disease, hypertension, and diabetes. This is a voluntary and confidential program.
Depending upon your health needs, you may qualify to earn up to $450 in pharmacy credit.
Members of Duke Select and Duke Basic are automatically enrolled in DukeWell. They can begin their active participation in the program by completing a simple health questionnaire called a Health Risk Assessment, or HRA. Employees and covered dependents 18 and older are eligible to participate.

Need Help Finding a Primary Care Physician?
DukeWell is committed to ensuring that all Duke Select and Duke Basic members have access to a Primary Care Provider. As a result, we have established partnerships with area physicians who will work with us to fast-track new patients into their practices.
