What is a Personal Health Plan?

DukeWell is intended to help you develop a Personal Health Plan. As a DukeWell participant, there are four steps to complete. These steps are:

Step 1: Assess your health. Assess your current health and identify any specific health issues you may face now or in the future by taking a Health Risk Assessment.

Step 2: Balance your lifestyle and set priorities for your health.

Step 3: Set goals for healthy actions. Review your Health Risk Assessment results and consider your personal health needs to set health goals in your Personal Health Plan. Your goals should be S.M.A.R.T. goals. That is, they should be:

SPECIFIC: Your goals should be clear and precise.
MEASURABLE: You must be able to measure your progress toward meeting your goals.
ACTION-ORIENTED: Your goals should include action-oriented behavior that is totally in your control.
REALISTIC: Your goals should challenge you but still be within your reach.
TIMED:You should set a time frame within which to evaluate your progress toward meeting your goals and a time by which your goals will be accomplished.

Step 4: Track progress toward meeting your health goals. Document your Personal Health Plan goals at the end of your Health Risk Assessment and track your progress in meeting your goals, using your Personal Health Tracker.
