Basic Group Life: The $10,000 Basic Group life policy
may be converted to a whole life individual policy within 30 days of retirement.
Otherwise, the coverage terminates. Forms requesting conversion are available
through Mark III Brokerage, Inc., 211 Greenwich Road, Charlotte, NC 28211-2337.
You may contact them at 1-800-532-1044. You will be direct billed the applicable
premium.
Supplemental Life: At the time of retirement, if you have been enrolled in
Supplemental Life insurance for at
least two years and wish to continue coverage, contact Marsh@ Work Solutions
at 1-800-552-9670. You may continue your current coverage at the retiree group
rate, or reduce coverage in
increments of $10,000. Coverage may be continued until age 95. You will be
direct billed the applicable premium on a quarterly basis. Universal Life: If you are enrolled in one or both of
the Universal Life plans, contact your
provider(s) for details about your options. The providers are the Holroyd Agency
at 1-800-582-1879 and North Carolina Mutual at 1-866-533-2357.
