Report a Work-Related Accident, Injury or Illness
If you have an accident, are injured at work, or develop an illness that you believe to be related to your job, report it by the end of your shift by completing the incident report you'll find below. Certainly seek medical treatment from one of the approved medical providers if the injury or illness is severe; but always follow up by filling out the incident report. Your information about the incident is important in helping Duke evaluate the circumstances of your incident and develop strategies for prevention of reoccurrences.
