Dental Insurance
Duke has an expanded network of providers who have agreed to accept usual and customary (U&C) for covered services. Under the dental plans, participants are able to visit any licensed dentist. You can find a network provider by calling Ameritas at 1-800-487-5553 or by visiting the Ameritas web site.
Vision Care Insurance
To find a network provider, please visit the UnitedHealthcare Vision web site or call 1-800-638-3120.
Mental Health & Substance Abuse Benefits
To find a network provider, call CIGNA Behavioral Health toll-free at 1-888-253-8552 or log on to their web site (Employer ID: Duke, PIN: employee). You can also browse a list of Duke mental health providers that are considered network providers for Duke employees and their dependents
