Long Term Care Insurance
Eligibility
You are eligible to apply for coverage under the plan if you are
an employee or House staff member and scheduled to work at least 20
hours per week and are included in a benefit eligible classification.
In addition, an eligible dependent of an employee or House staff
member may also apply for coverage. An eligible dependent includes
an employee's or house staff member' spouse, same sex spousal equivalent,
parents, parents-in-law, grandparents or grandparents-in-law who
have not attained age 90.
Eligibility to enroll is lost if your employment changes to an
ineligible status. You may continue the insurance by paying the
premium for the coverage while you are on an unpaid leave of absence,
sabbatical, or lay off status. You may also continue the insurance
at retirement or termination by paying the premiums directly to
CNA. Coverage may also be lost for failing to pay the premium.
Additional circumstances which may result in your ineligibility
for benefits or loss of coverage under the plan are outlined in
the certificate of insurance issued by CNA.
When Coverage Begins
Coverage begins on the first day of the month coincident with
or next following entry into the class of employees eligible for
coverage under the plan; provided the enrollment form for such
employee is received by CNA within 60 days after becoming eligible
to apply for coverage. If an eligible employee, House staff member,
spouse or same sex spousal equivalent enrolls more than 60 days after
becoming eligible to apply for coverage under the plan, coverage will
begin on the first day of the month coincident with or next following
the date the insurance company approves the application. Coverage
for an eligible parent, parent-in-law or grandparent-in-law of an
eligible employee will begin on the first day of the month following
the date the insurance company approves the application.
Cost of the Plan
Participants pay the entire premium normally through payroll deduction.
The amount is determined by the insurance company.
