by one of Duke's four health plans is more than $3,100, but
with Duke's health benefits, participants only pay a small
portion of that cost.

Coverage & Eligibility
Whom Can I Cover?
When making your health care, dental care, and vision care selections, you may
choose from among the following levels of coverage:
- Employee
- Employee/Child
- Employee/Children — only available for health and vision care*
- Employee/Spouse or Employee/Same-Sex Spousal Equivalent
- Family (includes Spouse)
Your premium for coverage — including your contribution and Duke's
contribution towards care — will vary, depending on which level of coverage you
select. Your Personal Information Fact Sheet provides you with the premium
amounts for each level.
*Dental care coverage does not include an Employee/ Children option. You may cover any
number of children in the dental plan by choosing the Family option.
Who Are My Eligible Dependents?
The following dependents are eligible for enrollment:
- Your legal spouse or same-sex spousal equivalent
- Your unmarried dependent children (includes natural children, stepchildren,
adopted children, foster children, or children for whom you are a legal
guardian up to age 19, or up to the 26th birthday if unmarried, full-time
students. Dependent children do not include grandchildren, siblings, or
other family members, or children of whom you have legal custody)
- Your children who are mentally or physically disabled and incapable
of self-support after age 19, as long as their disability began before they
turned age 19, and they were covered under a Duke health plan prior
to age 19
Your contributions for health care, dental
care, and vision care are deducted from
your pay on a pre-tax basis, saving you
federal income, state income, and Social
Security taxes. In some circumstances,
however, the federal tax law may require
post-tax treatment.
