Placement Health Review Requirements

The placement health review is a required process for selected job positions at Duke University based on federal, state and University regulations and policies. The basic requirements are described below by job group. This listing is not exhaustive nor does it preclude additional interventions based on individual and job exposure variation. Placement health review information is maintained confidentially and separate from personnel and other Duke medical records.

  • Testing and vaccination are available without cost to employees at Duke EOHW.

Faculty and Staff with Patient Contact

  • Health history review — Job pertinent health history information is collected.
  • Substance Abuse Screening — All Health System employees will be screened for substance abuse. Must bring picture ID for this.  
  • Tuberculosis screening
    • Tuberculin status is required.
    • Testing within 90 days of hire is required.
    • History of positive tuberculin reaction requires documentation
    • BCG history does not preclude tuberculin testing.
    • Documentation of, or a chest-ray at least 2 years after tuberculin test conversion is required. New tuberculin conversion requires current chest x-ray.
    • Recent tuberculin conversion requires current chest x-ray

Immunity status

Immunity to measles, mumps, rubella, polio, and chickenpox is required. The vaccine for Diphtheria, Tetanus, Acellular Pertussis (Tdap) is required for all faculty and staff who provide direct patient care to children 18 months of age or younger. History of disease is acceptable for measles, mumps, and varicella if born before 1/1/57; otherwise only the following is acceptable:

  • Immunity to Measles — Must provide official documentation of one of the following:
    • Documentation from health care provider of a diagnosis of measles disease
    • Lab evidence of measles immunity (blood test)
    • Two doses of live measles-containing vaccine on or after the 1st birthday
  • Immunity to Mumps — Must provide official documentation of one of the following:
    • Documentation from health care provider of a diagnosis of mumps disease
    • Lab evidence of mumps immunity (blood test)
    • Two doses of live mumps-containing vaccine on or after the 1st birthday
  • Immunity to Rubella — Must provide official documentation of one of the following:
    • Lab evidence of rubella immunity (blood test).
    • One dose of live rubella-containing vaccine on or after the 1st birthday
  • Immunity to chickenpox (Varicella Zoster) — Must provide official documentation of one of the following:
    • Documentation from health care provider of a diagnosis of chickenpox disease.
    • Lab evidence of varicella immunity (blood test).
    • Two doses of Varicella Vaccine.
  • Immunity to Pertussis-- Must provide official documentation of vaccination if providing care to children 18 months of age or younger.
  • Immunity to Polio
    • History of receiving polio vaccine

Color Blind Screen

All clinic staff (MD, PA, NP, RN, MLPN, CNA, Lab Tech) who might do point of care testing are required to have a color blind screen. Documentation of prior testing is acceptable.

Hepatitis B Vaccination

Vaccination for Hepatitis B is strongly encouraged. Bring record of vaccination and/or Hepatitis B Antibody results.

  • Respiratory Clearance Health Form — Respirator health history information is collected.
  • Respiratory fit testing is required of selected job positions.

New Hires to the Health System with No Patient Contact

  • Health history review — Job pertinent health history information is collected.  
  • Substance Abuse Screening — All Health System employees will be screened for substance abuse. Must bring picture ID for this.

Faculty and Staff Working with Research Animals

  • Health history review for Animal Handlers — This can be completed and submitted online from this web site. Job pertinent health history information is collected.
  • Those working with non human primates must have tuberculosis screening and immunity to measles.
    • Tuberculosis screening
      • Testing within 90 days of hire is required.
      • History of positive tuberculin requires documentation
      • BCG history does not preclude this requirement
