Employment Eligibility Verification Form (federal). If you have Adobe Acrobat 4.0 or higher, you can complete this form online before printing. Instructions are included in this form. Forms that do not bear an original signature will not be accepted. The I-9 Form, the Background Check/Certification Form and appropriate tax forms should be sent to the HR Information Center at 705 Broad Street (Box 90496), Durham, NC 27705 (fax number 919-668-0386) once the Hire iForm is completed.
